Today an elderly patient came in without her insurance card. MyChart wasn’t set up, and her caregiver was doing their best to fill in the blanks. It reminded me how important it is to keep our medical information organized — for ourselves, our family, and those who help care for us.

Whether you’re a patient, a family member, or a caregiver, having a personal health organizer can turn confusion into calm.

1️⃣ Start with a File Folder or Binder

Choose what works best for you — an accordion file or a colorful binder with dividers. Make it easy to carry and easy to open.

2️⃣ The Front Pocket: Insurance and ID

Keep these at the very front for quick access:

Insurance card(s) — medical, dental, vision Photo ID Emergency contacts

3️⃣ Doctor Directory

Create a page for each provider:

Name and specialty Clinic name, address, and phone number Fax or email if available Notes from visits or reminders

Keep the most current list up front — it’s one of the most important parts.
